Hip Resurfacing Surgery:
Hip resurfacing is a surgical procedure (a form of arthroplasty) which has been developed as an intervention alternative to total hip replacement (THR). The potential advantages of hip resurfacing include less bone removal (bone preservation), a potentially lower number of hip dislocations due to a relatively larger femoral head size, and possibly easier revision surgery for a subsequent total hip replacement device because a surgeon will have more bone stock available to work with. The potential disadvantages of hip resurfacing are femoral neck fractures (rate of 0-4%), aseptic loosening, and metal wear.

Hip Resurfacing surgery has a 99% success rate. Many adolescent patients have had the Hip Resurfacing procedure with excellent results. Procedure can be done soon after skeletal maturity. Following Hip Resurfacing, there is faster rehabilitation and early discharge from hospital.
Indications for Hip Resurfacing Surgery:
The indications for Hip Resurfacingsurgery include avascular necrosis of hip bone, primary or secondary osteo-arthritis, arthritis following hip fractures, ankylosing spondylitis, long standing perthes disease or a dysplastic hip.
Candidates for Hip Resurfacing Surgery:
The Hip Resurfacing surgery is ideal for younger or active patients considering hip replacement surgery as a treatment for hip pain or hip arthritis. Hip resurfacing is especially relevant to patients concerned about bone conservation.
Many hip pain sufferers under the age of 60 will outlive a traditional hip replacement and require a second, more difficult hip replacement surgery. For such patients, hip resurfacing surgery might be the best route. Many active patients over 60 years old are also good Hip resurfacing candidates, following review by a physician.
Inactive or elderly men and women are not ideal candidates for hip resurfacing surgery. Also, some chronic forms of arthritis severely deform the hip bones. Hip resurfacing may not be the best option for such patients.

Preparation for Hip Resurfacing Surgery:
You and your orthopedic surgeon may participate in an initial surgical consultation. This appointment may include pre-operative X-rays, a complete medical and surgical history, physical examination, and a comprehensive list of medications and allergies. During this visit, your orthopedic surgeon will likely review the procedure and answer any questions.
Your orthopedic surgeon may require that you have a complete physical examination by your internist or family physician, as you will need to be cleared medically before undergoing this procedure. Your surgeon may suggest that you consider donating your own blood to save in case you require it during surgery or in the event of a post-operative blood transfusion.
You should follow your regular diet on the day before your surgery.
Do not eat or drink after midnight the night before surgery. On the morning of surgery, you may brush your teeth and rinse your mouth, but do not swallow any water.
Follow your doctor's instructions regarding use of medication in the days leading to surgery. In some cases, a blood thinner may be ordered a few days before surgery.
Generally, aspirin and non-steroidal anti-inflammatory medications should not be taken seven days prior to surgery.
Try to get long, restful nights of sleep. A sleeping medication may be ordered the evening before surgery.
Hip Resurfacing Surgery Procedure:
An incision is made over the hip to expose the hip join
The acetabulum (socket) is prepared using a special instrument called a reamer.
The acetabular component is then inserted into the socket. This is sometimes reinforced with screws or occasionally cemented.
A liner which can be made of plastic, metal or ceramic material is then placed inside the acetabular component
The femur (thigh bone) is then prepared. The femoral head which is arthritic is cut off and the bone prepared using special instruments, to exactly fit the new metal femoral component.
The femoral component is then inserted into the femur. This may be press fit relying on bone to grow into it or cemented depending on a number of factors such as bone quality and surgeon's preference.
A trial reduction (putting the hip back into place) is performed to make sure everything fits well.
The real femoral head component is then placed on the femoral stem. This can be made of metal or ceramic.
The hip is then reduced again, for the last time. The muscles and soft tissues are then closed carefully.
Recovery after Hip Resurfacing Surgery:
If you need pain relief, you can take over-the-counter painkillers such as paracetamol or ibuprofen. Always read the patient information that comes with your medicine and if you have any questions, ask your pharmacist for advice.
The exercises recommended by your physiotherapist are a crucial part of your recovery, so it's essential that you continue to do them.
There are certain movements that you shouldn't do in the first eight weeks. For example, you shouldn't cross your legs or twist your hip inwards and outwards. Your physiotherapist will give you further advice and tips to protect your hip.
You should be able to move around your home and manage stairs. You will find some routine activities, such as shopping, difficult for a few weeks and will need to ask for help. You will need to use crutches for about four to six weeks.
You may be asked to continue wearing your compression stockings for a few weeks at home.
You can usually return to light work after about six weeks. But if your work involves a lot of standing or lifting, you may need to stay off for longer.
Follow your surgeon's advice about driving, as the length of time before you are fit to drive will depend on several factors, including which leg has been operated on and whether your car is automatic.
Advantages of Hip Resurfacing Surgery:
Femoral head is preserved.
Femoral canal is preserved and no associated femoral bone loss with future revision. Also, the risk of microfracture of femur with uncemented stem implantation is eliminated.
Larger size of implant "ball" reduces the risk of dislocation significantly.
Resurfacing patients are more likely than total hip replacement patients to recover a natural gait.
Stress is transferred in a natural way along the femoral canal and through the head and neck of the femur. With the standard THR, some patients experience thigh pain as the bone has to respond and reform to less natural stress loading.
Use of metal rather than plastic reduces osteolysis and associated early loosening risk.
Use of metal has low wear rate with expected long implant lifetime.
Risks of Hip Resurfacing Surgery:
There is some risk of the femoral neck breaking. This risk can be nearly eliminated by proper patient selection, correct angle of implantation of the device and avoiding notching the neck when preparing the femoral head in surgery.
Despite known low wear rate, longevity and long-term effects of wear debris are unknown.
